Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility file review the LPA reviewed a fire drill log that documented the last fire drill conducted on 7-4-2025.

Resolution: Licensee understands a fire drill must be conducted at least once every six months. Licensee agrees to conduct a fire drill and submit documentation to the Department on or by POC due date of 1-30-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health and safety risk to the children in care. During the staff file review the LPA was unable to review a current Mandated Reporter certificate for S1 or S2.

Resolution: Licensee understands all staff working with children must have a current Mandated Reporter certificate on file and available for review during the inspection. Mandated Reporter training must be taken every two years. Licensee agrees to have S1 and S2 complete the training and submit the certificate of completion to the Department on or by POC due date of 1-30-2026. Training can be taken at www. During the staff file review the LPA was unable to review a current EMSA approved CPR/1st Aid certificate for S1 or S2. EMSA approved trainings can be found at www. emsa.ca.gov

Resolution: Licensee understands there must be at least one staff present with a valid EMSA approved CPR/1st Aid certificate on file and available for review. Licensee agrees to have S1 and S2 complete the training and submit the certificate of completion to the Department on or by POC due date of 1-30-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health and safety risk to the children in care. During the facility file review the LPA was unable to review a current facility roster with all children present.

Resolution: Licensee understands all children must be on the facility roster upon enrollment. Licensee updated the roster during the inspection.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the facility records review the LPA was unable to review a current roster. The roster was missing four children who were present during inspection.

Resolution: Licensee understands all children enrolled must be documented on the LIC 9040 Child Care Facility Roster. During inspection Licensee updated the facility roster.

Resolution: Licensee agrees to submit immunization records for C2 to the Department on or by POC due date.
