Based on the record review, the Licensee did not meet above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. Upon arrival to the facility the Licensee disclosed that she has not received a clearance for S1. During record review LPA discovered S1 does not have a clearance and is not associated to the facility.
Resolution: Licensee understands S1 cannot work, reside or volunteer at the facility until a Criminal Record Clearance is obtained and S1 is associated to the facility. Licensee agrees to submit required documentation to Licensing in order to start the clearance and association process. Licensee agrees to provide an LIC 9224 to all enrolled families for signature and place in their file for 12 months. Licensee understands all children who enroll within the next 12 months are required to have the LIC 9224 on file.
Based on the record review, the Licensee did not meet above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During required it was observed the Licensee's Mandated Reporter certificate expired.
Resolution: Licensee agrees to complete training and submit the completion of training certificate to the Department on or by the POC due date.
