Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. Licensee showed a video to the LPA of when she conducted a fire drill on 4-15-2025 but did not document it.

Resolution: Licensee understands all fire drills must be documented. Licensee documented the fire drill during the inspection.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review LPA was unable to review a current Mandated Reporter certificate for S2. Training can be completed at www. Mandatedreporterca .com

Resolution: Licensee agrees to have S2 take the training and submit a copy to the Department on or by POC due date of 6-15-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review immunizations records for S2. Licensee stated she was unaware S2 was required to have immunizations.

Resolution: Licensee agrees to obtain immunizations (MMR & DTap) for S2 and submit to the Department on or by 3-31-2024.

Based on the record review, the Licensee did not meet Operation of a Family Child Care Home which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review LPA reviewed a fire drill log with a date of 12-25-2022. Licensee stated she has not completed a fire drill since she reopened.

Resolution: Licensee agrees to conduct and submit a current fire log on or by the POC due date.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review LPA was unable to review a Mandated Reporter certificate. Licensee stated she does not have a copy available for review.

Based on the record review, the Licensee did not meet Personnel Requirements which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review LPA reviewed a CPR/1st Aid that expired on 7-14-2021. Licensee stated she does not have a current CPR/1st Aid card available for review.

Resolution: Licensee agrees to submit a current CPR/1st Aid to the Department on or by POC due date.
