Based on observations, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure mandated reporter training was completed by the expiration date which poses a potential safety risk to children in care.

Resolution: LPA reminded Licensee to ensure all staff renew the mandated reporter training course every two years. Licensee agreed to provide completed certificate for S2 to LPA by 05/15/2024.

Resolution: LPA discussed the requirement to maintain documentation of the required immunizations for all employees and volunteers. Licensee agreed to provide proof of required immunizations for S3 to LPA by 05/15/2024.
