Based on the observation, the Licensee did not meet the above regulation which poses an immediate health & safety risk to the children in care. During the facility tour the LPA observed S2 alone with nine children which included three infants and six toddlers.

Resolution: Licensee acknowledged being out of ratio. Licensee understands when there are more than 8 children present there must be two qualified staff. If two qualified staff are not present, the staffing ratio and capacity of a small family child care must be followed to be in compliance. Licensee shall submit a written statement of understanding to the Department on or by POC due date on 1-12-2026. Licensee also agrees to provide an LIC 9224 as it states in this report.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility tour the LPA reviewed a fire drill log with the last fire drill conducted on 6-16-2025.

Resolution: The Licensee understands a fire drill must be conducted at least once every six months. Assistant agrees to forward this report to the Licensee. Licensee shall conduct a fire drill, document and submit the log to the Department on or by POC due date of 1-15-2026.

Based on the observation and interview, the Licensee did not meet the above regulation which poses a potential health, safety & personal rights risk to the children in care. Upon arrival the LPA was informed by the Licensee that she was not present and would not return through the duration of the inspection.

Resolution: Licensee understands Licensees are permitted to be away from the facility temporarily. A temporary absence shall not exceed 20 percent of the hours when care is provided. Licensee shall submit a written statement of understanding to the Department on or by POC due date of 1-15-2026.

During the children’s file review the LPA reviewed sleep logs for C1, C2, C4 that were timed for every 30 minutes. All sleep logs had set times such as 12:15PM, 12:45PM, 1:15PM, etc.

Resolution: Licensee understands sleep logs must be conducted every 15 minutes and the times must be accurate. If a child falls asleep at 12:04PM, the next time should be 12:19PM, etc. Licensee shall submit a written statement of understanding to the Department on or by POC due date of 1-9-2025.

Based on the record review, the Licensee did not meet the above regulation which poses an a potential Health, Safety & Personal Rights risk to the children in care. During record review, LPA reviewed a CPR/1st Aid card for S1 that is not an EMSA approved vendor. LPA also reviewed a CPR/1st Aid card for S2 that did not have both required stickers.

Resolution: Licensee agrees to have S1 complete CPR/1st Aid training with an EMSA approved vendor and submit a current certificate to the Department on or by the POC date of 11-27-2023. Licensee also agrees to obtain both required stickers for S2.
