Based on the interview, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During interview the Licensee stated on 12-11-2023 two adult residents moved in and have not obtained a Criminal Record Clearance. Civil Penalty of $1000 assessed.
Resolution: Licensee understands all adults over the age of 18 must obtain a Criminal Record Clearance prior to working, residing, or volunteering in a licensed facility. Licensee stated she will take both residents over the weekend to complete a Live Scan and email the LIC 9163 to the Department on or by POC due date of 12-18-2023.
Based on the record review, the Licensee did not meet the above regulation which poses an potential Health, Safety & Personal Rights risk to the children in care. During record review LPA reviewed expired Mandated Reporter certificates for the Licensee and S2.
Resolution: Licensee understands Mandated Reporter training must be completed every two years. Licensee agrees to complete Mandated Reporter training and have S2 complete Mandated Reporter training. Licensee agrees to submit a current completion of training certificate to the Department on or by POC due date of 1-3-2024.
