Based on the record review, the Licensee did not meet the above regulation which poses a potential Health and Safety to the children in care. During the staff file review the LPA was unable to review a current Mandated Reporter certificate for S2.

Resolution: Licensee understands all staff must have a current Mandated Reporter certificate on file and available for review. The training must be completed every two years and can be completed on www. Mandatedreporterca. com. Certificate of completion is due on or by the POC due date of May 30, 2025.

Based on the observation and record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During a facility tour the LPA observed a pool alarm which was tested by the Licensee and was in working condition. However, the pool alarm was not ASTM approved.

Resolution: Licensee ordered ASTM approved pool alarms during the inspection and will send a video to the Department on or by the POC due date of 5-30-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review a current CPR/1st Aide card for S1 or S2.

Resolution: Licensee understands that at least one staff must have an EMSA approved CPR/1st Aid certificate on file and available for review. Licensee understands CPR/1st Aid must be taken every two years. Licensee agrees to complete CPR/1st Aid training and submit certificate of completion to the Department on or by POC due date of 5-31-2024.
