Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the review of the facility documents, the LPA was unable to review a current fire drill. The last fire drill was conducted on 12-22-24.

Resolution: Licensee understands a fire drill must be contacted at least once every six months. Licensee agrees to conduct a fire drill and submit documentation to the Department on or by POC due date of 12-27-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the staff file review, the LPA was unable to review a current Mandated Reporter certificate for S2 and S3. S2 stated the trainings were completed, however, the certificates could not be pulled up on the computer.

Resolution: Licensee understands current Mandated Reporter training certificates must be available for review during inspections. Licensee agrees to submit current certificates to the Department on or by POD due date of 2-27-2026. Training can be completed at www. Mandatedreporterca .com

Based on the record review, the Licensee did not meet the above regulation which poses a potential health & safety risk to the children in care. During the children’s file review, sleep logs for C2.

Resolution: Licensee understands all children under the age of two must have a 15 minute sleep log available for review during the inspection. Licensee agrees to submit a written statement of understanding of the regulation. Regulations were provided during inspection. Statement is due on or by POC due date of 2-27-2026.

Based on the record review, the Licensee did not meet the above regulation which poses an immediate safety risk to the children in care. During the review of facility associations, the LPA was unable to review a clearance for R1. $500 Civil Penalty assessed

Resolution: Licensee understands that all adults residing, working or volunteering over the age of 18 must have a criminal record clearance and be associated to the facility. Licensee agrees to have R1 obtain a criminal record clearance and submit proof of fingerprints to the Department by close of business on 1-10-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunization for C1.

Resolution: Licensee understands all children enrolled must have current immunizations on file and available for review. Licensee agrees to obtain immunizations for C1 and submit to the Department on or by 1-15-2025.
