Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file the LPA was unable to review sleep logs for C1.
Resolution: Licensee understands all children up to 24 months must have sleep logs completed every 15 minutes until the child wakes up and documentation must be kept on file for review. Licensee agrees to write a statement of understanding. Safe Sleep regulations were provided during inspection.
Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file the LPA was unable to review immunizations for C3. This is a repeat violation. $250 civil penalty assessed.
Resolution: Licensee understands all children enrolled must have immunizations on file during the time they are enrolled. Licensee agrees to obtain the immunizations for C3 and submit a copy to the Department on or by POC due date of 10-30-2024.
Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file the LPA was unable to review an LIC 627 Consent for Emergency Medical Treatment form for C1 and C5.
Resolution: Licensee understands all children enrolled must have required Licensing forms on file and available for review. Licensee agrees to obtain the LIC 627 for C1 and C5 then submit a copy to the Department on or by POC due date of 10-30-2024.
Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file the LPA was unable to review an LIC 995A Parents Rights for C5.
Resolution: Licensee understands all children enrolled must have all required Licensing forms on file and available for review. Licensee agrees to obtain an LIC 995A for C5 then submit a copy to the Department on or by POC due date of 10-30-2024.
