Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file reviews the LPA was unable to review a liability insurance affidavit for C1 and C4.

Resolution: Licensee understands if she does not have a current liability insurance policy, an affidavit must be on file. Licensee agrees to obtain a signed affidavit for C1 and C4 and submit to the Department on or by POC due date of 3-3-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review the LPA was unable to review MMR vaccinations for S2.

Resolution: Licensee understands all staff must have all required vaccinations. Licensee agrees to have S2 obtain an MMR vaccination. Licensee agrees to submit proof of vaccination to the Department on or by POC due date of 1/3/2024.

During the record review the LPA was unable to review a file for C2. The Licensee stated there was not a file available for review during the inspection.

Resolution: Licensee agrees to obtain the required documents with signatures and dates for C2. The Licensee will submit the required documents to the Department on or by 10/27/2021.

During the record review the LPA was unable to review an LIC 995A for C2 and C6. The Licensee stated there was not an LIC 995A available for C2 and C6 during the inspection.

Resolution: Licensee agrees to submit an LIC 995A for C2 and C6. The Licensee agrees to submit the LIC 955A for both children to the Department on or by 10-27-2021.
