ELDRIDGE FAMILY CHILD CARE

Based on the observation, the Licensee did not meet the above regulation which poses an immediate health & safety risk to the children in care. During the facility tour the Licensee did not have a working carbon monoxide detector. During the inspection the Licensee found a working detector in the kitchen drawer which she understands must remain plugged in or a battery must be installed.

Resolution: Corrected during inspection

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review a Mandated Reporter Certificate for S2.

Resolution: Licensee understands all individuals working with children must have a Mandated Reporter certificate. Licensee agrees to have S2 complete training and submit the certificate of completion to the Department on or by POC date of 9-19-2025.

Based on the record review, the Licensee did not meet the above regulation which poses an potential Health risk to the children in care. During the staff file review the LPA was unable to review MMR for S2.

Resolution: Licensee understands all individuals must have required vaccinations (MMR/DTap) and proof on file during the inspection. Licensee agrees to have S2 obtain proof of vaccination and submit to the Department on or by POC due date of 9-30-2025.

Based on the record review, the Licensee did not meet the above regulation which poses an potential health risk to the children in care. During the children’s file review the LPA was unable to review immunizations for C4.

Resolution: Licensee understands all children must have immunizations prior to starting care. Licensee agrees to obtain proof of vaccination and submit to the Department on or by POC due date of 9-19-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the review of children’s file the LPA was unable to review the required documents for C2 who was present during the inspection.

Resolution: Licensee understands all children must have a file available for review during inspection. Licensee agrees to obtain required documents for C2 prior to C2 returning to care. Licensee agrees to submit required documentation to the Department on or by POC due date of 9-19-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file review the LPA was unable to review sleep logs for C1.

Resolution: Licensee understands all children up to the age of 24 months must have sleep logs on file. Sleep logs are 15 minute checks every time the child is sleeping at the facility and includes date and the name of the child. Sleep logs should be accurate timing not preset times such as 12:00, 12:15, 12:30. Licensee agrees to submit a written statement of understanding and submit to the Department on or by POC due date of 9-16-2025.
