Based on the observation, the Licensee did not meet the above regulation which poses an immediate health risk to the children in care. Upon arrival the Licensee was observed outside. Upon arriving in the home there were three children in who were watching TV. Two were school age and one baby. During the facility tour the LPA observed a knife in the sink and cleaning products on the kitchen counters.

Resolution: Licensee understands all hazardous items, including but not limited to, knives and cleaning products should be made inaccessible to the children in care. Licensee agrees to submit a written statement of understanding of the regulation. Statement is due on or by POC due date of 9-22-2025.

Based on the observation, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility tour the LPA observed stairs that were not barricaded with a child under age of five present.

Resolution: Licensee understands stairs must be barricaded anytime there are children under the age of five present in care. Licensee agrees to submit a written statement of understanding of the regulation. Statement is due on or by POC due date of 9-24-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review a current Mandated Reporter training certificate for S1.

Resolution: Licensee understands all staff working with children must have a current Mandated Reporter certificate available for review. Manadated Reporter training must be renewed every two years. Training can be taken at www. Mandatedreporterca .com. Licensees agrees to have S1 complete the training and submit the certificate to the Department on or by POC due date of 9-24-2025.

Based on the observation, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During facility tour LPA observed stairs were not barricaded with children under the age of five present.

Based on the observation, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During facility tour the LPA observed a propane tank on the side of the house, cleaning products under the cabinet and items labeled “Keep out of reach of children” on the counter in the restroom children use. In addition, LPA observed animal feces in the backyard where children play. During inspection children were sleep.

Resolution: Licensee understands all hazardous items must be kept out of reach of children and all areas must be free of animal feces. Licensee agrees to remove propane tank and clean backyard prior to children using the areas. Licensee agrees to send pictures on or by POC due date of 6-15-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health risk to the children in care. During review of staff files the LPA was unable to review a CPR/1st Aid card for S2. Upon arrival S2 was alone providing Supervision & Care to the children in care.

Resolution: Licensee understands at least one staff present must have current CPR/1st Aid training and card must be on file. Licensee agrees to have S2 complete training and agrees to submit certificate to the Department on or by POC due date of 6-30-2024.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During review of children files the LPA was not able to review all of the required forms for C1, C2, and C4. Required forms include, but not limited to, LIC 700, LIC 627, LIC 995, LIC 282, LIC 9150, current immunizations.

Resolution: Licensee agrees to submit missing forms for C1, C2 and C4 on or by POC due date of 6-15-2024 and will ensure any children enrolled have all required forms on file in the future.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During review of C2 file LPA was unable to review an LIC 9227.

Resolution: Licensee understands an LIC 9227 is required for all children under the age of 12 months. Licensee agrees to obtain and submit an LIC 9227 then submit to the Department on or by POC due date of 6-15-2024. Safe Sleep regulations provided during inspection.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During review of C2 file LPA was unable to review sleep logs.

Resolution: Licensee understands sleep logs are required for all children under the age of 24 months. Licensee agrees to document every 15 minutes every time C2 sleeps until 6-29-2024 then submit sleep logs on 6-30-24. Safe Sleep regulations provided during inspection.
