Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the staff file review, the LPA was unable to review a TB clearance for S3.

Resolution: Licensee understands all adults working with the children in care must have a TB clearance on file. Licensee agrees to have S3 take a TB test, obtain a clearance letter and submit it to the Department on or by POC due date of 3-10-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review, the LPA was unable to review a Mandated Reporter certificate for S3.

Resolution: Licensee understands all adults working with the children in care must have a Mandated Reporter certificate on file and available for review. Mandated Reporter training must be completed every two years. Training can be taken at www. Mandatedreporterca .com. Licensee agrees to have S3 take training and submit the certificate to the Department on or by POC due dated of 3-3-3026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review, the LPA was unable to review immunizations for C4.

Resolution: Licensee understands all children must have immunizations and records must be available for review. Licensee agrees to obtain immunization records for C4 and submit to the Department on or by POC due date of 3-3-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety & personal Rights risk to the children in care. During the children’s file review, the LPA was unable to review LIC 9150 Parent Notification Additional Children In Care for 5 of 5 files reviewed.

Resolution: Licensee understands all children enrolled must have an LIC 9150 on file if she plans to enroll two school age children giving her the max capacity. Licensee agrees to submit a written statement of understanding of the regulation. Statement is due on or by POC due date of 3-3-2026.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file review, the LPA was unable to review a current sleep log for C3.

Resolution: Licensee understands all children under the age of 24 months must have a 15 minute sleep log on file and available for review. Licensee agrees to submit a written statement of understanding of this regulation. Statement is due on or by POC due date of 3-3-3026.
