Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During review of staff files the LPA was unable to review an EMSA approved CPR/1st Aid certificate for S2.

Resolution: Licensee understands all staff working alone with the children must have an EMSA approved CPR/1st Aid certificate on file and available for review during inspection. The Licensee agrees to have S2 complete an EMSA approved CPR/1st Aid training to remain in compliance during times the Licensee needs to be away from the facility. Certificate can be submit to the Department on or by POC due date of 2-28-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file review the LPA was unable to review immunizations for C1.

Resolution: Licensee understands all children enrolled must have immunizations on file and available for review during inspection. Licensee agrees to obtain immunization records for C1 and submit to the Department on or by POC due date of 2-20-2025.

Based on the observation and interview, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the inspection the LPA was informed and adult resident is living in the home who the LPA verified does not have a clearance and is not associated to the facility.

Resolution: Licensees understand all adults over the age of 18 residing, working or volunteering at the facility must have a fingerprint clearance and be associated to the facility. A statement of understanding and a plan to remain in compliance is due by close of business on 2-14-2025. Civil Penalty of $500 assessed.

Based on the observation and interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the inspection the LPA was informed Lakeisha Bolden, Licensee is not currently living at the facility. At the time of inspection Angela Bolden was not present.

Resolution: Licensees understand both Licensees must be living at the facility and be present 80% of the time. Licensee agrees to submit a statement of understanding and a plan to get back into compliance to the Department on or by POC due date of 2-20-2025.

Based on observations, interviews conducted, and records reviewed, the licensees did not comply with the section cited above as licensees did not ensure 15-minute sleeping logs were maintained for one of one child which poses a potential health, safety or personal rights risk to children in care.

Resolution: LPAs discussed the requirement to check and log infant sleeping every 15 minutes for infants under 24 months and to maintain all sleeping logs at the facility for review. Licensees agreed to provide LPAs with a completed 15-minute sleeping log for C1 by 02/15/2024.

Based on the interview/record review, the Licensee did not meet Immunizations regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the inspection the Licensee stated C1, C2 does not have immunizations available for review.

Resolution: Licensee agrees to submit immunization records for C1 and C2 by 9/30/2021. Licensee also agrees to ensure all children enrolled have immunizations on file.

Resolution: Licensee agrees to submit a completed file for C7 to the Department by 9/30/2021. Licensee also agrees to ensure all children have a file available for review with all required licensing forms.

Based on the interview/record review, the Licensee did not meet Safe Sleep regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the inspection the Licensee stated there is not an LIC 9227 available for review for C1

Resolution: Licensee agrees to submit an LIC 9227 for C1 to the Department by 9/24/2021. Licensee also agrees to ensure that all children who require an LIC 9227 have one on file.

Based on the interview/record review, the Licensee did not meet Safe Sleep regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the inspection the Licensee stated the infants are checked on while sleeping but there are no sleep logs available for review for C1, C2 and C3.

Resolution: Licensee agrees to send a sleep log to te Department by 9/24/2021. Licensee also agrees to ensure all children who require a sleep log have one on file.
