Based on the record review, the Licensee did not meet the above regulation which poses a potential Personal Rights risk to the children in care. During the children’s file review the LPA was unable to review and LIC 995 for C7.
Resolution: Licensee understands all children enrolled must have an LIC 995 on file and available for review during inspection. Licensee agrees to obtain an LIC 995 for C7 and submit to the Department by 1-30-2025.
Based on the observation, the Licensee did not meet the above regulation which poses an immediate health and Safety risk to the children in care. During the facility tour the LPA observed a carbon monoxide and smoke detector not inworking condition. Licensee stated both detectors need batteries.
Resolution: Licensee understands the facility must have a working carbon monoxide and smoke detector in the facility at all times. Licensee agrees to ensure there is at least one working carbon monoxide detector and one smoke detector in the home, then submit proof to the Department by close of business on 1-15-2025.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. Licensee stated she could not locate the fire drill.
Resolution: Licensee understands a fire drill must be conducted at least once every six months, must be documented and available for review during inspection. During facility records review the LPA was unable to review a fire drill. Licensee agrees to conduct a fire drill and send documentation to the Department by 1-30-2025.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety to the children in care. During the children’s file review the LPA was able to review sleep logs, however, the sleep logs were documented every 20, 30, 45 minutes.
Resolution: Licensee understands all children under the age of two must be checked every 15 minutes and it must be documented. Licensee agrees to conduct sleep logs for all children under the age of two from today until 1-29-2025 then submit documentation to the Department on 1-30-2025. Licensee understands sleep logs must be conducted and documented beyond 1-30-2025, until the child turns two.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file review the LPA was unable to review a file for C6.
Resolution: Licensee understands all children must have a file with all required documents on file and available for review during inspection. Licensee agrees to obtain all required documents and submit to the Department by 1-30-2025.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During children’s file review the LPA was unable to review immunizations for C2, C3, C5 and C6.
Resolution: Licensee understands all children enrolled must have current immunizations during their enrollment. Licensee agrees to obtain immunization records for C2, C3, C5 and C6 then submit to the Department by 1-30-2025.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility records review the LPA was unable to review a current roster.
Resolution: Licensee understands a complete and current roster must be available for review during inspection. Licensee agrees to complete the facility roster and submit to the Department by 1-30-2025.
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review LPA reviewed expired Mandated Reporter certificates for the Licensee and S2. Licensee stated there is no current certificate available and training has not been taken.
Resolution: Licensee understands Mandated Reporter must be completed every two years. Licensee agrees to complete training and have S2 complete training. Licensee agrees to submit certificates of completion to the Department on or by POC due date of 1/3/2024.
Based on the interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During interview Licensee a file is not available for C5.
Resolution: Licensee understands all children enrolled must have a file available for review during inspection. Licensee agrees to obtain all required documents including an LIC 9227 for C5. Licensee agrees to submit all required documents to the Department on or by POC due date of 1/3/2024.
Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review LPA reviewed a facility roster that did not include C4 and C5.
Resolution: Licensee understands all children enrolled must be on a facility roster. Licensee agrees to add C4 and C5 to the facility roster and submit the roster to the Department on or by POC due date of 1/3/2024.
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review LPA was unable to review sleep logs for C3. Licensee stated she was not aware the checks had to be documented.
Resolution: Licensee understands all children under the age of 24 months must have sleep logs available for review. Sleep logs include 15 min checks every time a child is sleeping. During inspection Licensee was able to find a “Sleep Check” option in Bright Wheel which she stated she will utilize. Licensee agrees to submit a written statement on or plan to ensure she is in compliance with the above regulation. The statement will be due on or by POC due date of 1/3/2024.
Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the facility tour LPA observed the only carbon monoxide detector without a battery.
Resolution: Cleared during inspection. Licensee understands carbon monoxide and smoke detectors must be in working condition at all times. During inspection a battery was installed, carbon monoxide detector was tested and in working condition. Licensee understands she must give all families enrolled and all families enrolled in the next 12 months an LIC 9224 which must be kept on file.
