Based on record review, the licensee did not comply with the section cited above in that LPA observed that Staff #4 and Staff #5 had missing licensing forms on file. LPA reviewed findings with Lead Teacher Staff #2 and Staff #5 which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in, LPA observed that there were no staff members on site that had a current CPR & FA on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above. Currently, the facility is utilizing Room 29, when it was previously licensed in Room 34. Licensing was not informed of this change, which poses a potential health, safety or personal rights risk to persons in care.

LPA observed that there are no exceedances. Yet, licensee did not have licensing forms or images on file for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.
