Based on observation, the Licensee did not comply with the section cited above. The children's bathroom cabinet has stored cleaning compounds, and the child-proof latch has worn out, which poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Licensees shall purchase and install new latches or move cleaning compounds from there, making them inaccessible to the children in care. The Licensee shall submit the proof of correction to the LPA via email.

Based on observation, interview, and record review, the Licensee did not comply with the section cited above. The Licensee did not have a complete disaster drill( fire and earthquake) log. Furthermore, LPA observed the Licensee updating the disaster drill log during the inspection, which poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Licensees to conduct a disaster drill (fire or earthquake) and, from now on, complete a drill at least once every six months.

Based on observation, interview and record review, the licensee did not comply with the section cited above as the facility does not have a fire extinguisher 2A-10-BC or higher which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Purchase a fire extinguisher that is 2A-10-BC or higher

Based on observation, interview and records reviewed, the licensee did not comply with the section cited above in that the licensee and assistant S2 do not have current Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Assistant S2 must take and complete the required Mandated Reporter training at mandatedreporterca.com

Based on observation, interview and records reviewed, the licensee did not comply with the section cited above in that the licensee and assistant S2 do not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create complete files for both the licensee and assistant S2

Based on observation, interview and records reviewed, the licensee did not comply with the section cited above in she does not have a certificate of completion of Preventative health practices which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will register/pay for Preventative Health and Safety training or provide copy of certification of completion of Preventative Health and Safety training at Admas Safety or contact Hively.
