Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. Licensee's CPR and first aid is expired since 02/26/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee shall complete CPR/first-Aid training and submit proof of completion to the department by POC due date. Going forward, this certificate must be renewed every two years.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. At 12:15pm, LPA observed that licensee was supervising four (4) infants and three (3) preschool children by herself. Although Licensee is licensed for a capacity of 14, the capacity reverts back to a capacity of 8 chilldren if a qualified assistant is not available, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee to send a written plan to the Department detailing how she will remain in compliance with the limitations of her license regarding capacity and ratio by POC due date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 5 childrens files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: licensee must have parents of C1 and C3 complete and sign the LIC 28 form and place in file

Based on observation and record review, the licensee did not comply with the section cited above in 4 out of 5 children files which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Have parents sign correct LIC 627 for C1, C3, C4 and C5

Based on observation, interview and record review, the licensee did not comply with the section cited above for licensee which poses a potential health, safety or personal rights risk to persons in care.

Resolution: licensee must complete or sign up for CPR/First Aide certification (EMSA approved)
