Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA osberved that S2 does not have required immunization ( measles and TB) on the file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall obtain immunization records for her helper and submit copies of documenation to LPA by the POC due date.
Based on observation,interview, and record review, the licensee did not comply with the section cited above. The review of drill log indicates that the last drill was conducted 12/07/2021. This poses a potential health and safety risk to children in care.
Resolution: Licensee shall submit proof of correction by the aforementioned date. Failure to follow through with this plan of correction will result in a civil penalty of $100 per day until corrected.
