Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above as the facility does not maintain 15-minute sleep logs for the infants, as required, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Effective immediately, the licensee shall conduct and document 15-minute sleep checks for all infants, including monitoring for signs of distress and labored breathing. The completed sleep logs shall be submitted to the Licensing Program Analyst (LPA) by the Plan of Correction (POC) due date.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above as facility does not have updated roster, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall update the roster and proof of compliance shall be submitted to the LPA by the due date.
Based on observation, interview,record review, the licensee did not comply with the section cited above. At 12:40pm, LPA observed five( 5) infants and eight (8) preschoolers present in the home simutaneously, placing the facility out of the ratio which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee called C13's parents and sent him home immediately. LPA discussed ratio requirements for a large Family Child Care Home with Licensee.
