Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. Upon arrival,LPA observed two infants sleeping in dining chair, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee shall submit a written statement of her understanding of the infant safe sleep regulations to the LPA.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above.Upon arrival, the Licensing Program Analyst (LPA) observed a helper alone supervising 10 children while the licensee had left the facility to drop off her own children at school. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA discussed large family child care ratio requirements with the licensee. If no assistant is present, the licensee must comply with the capacity limits of a Small Family Child Care Home, which allows a maximum of 8 children.The licensee shall submit a written statement of her understanding of these requirements to the LPA via email.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above as no records have been maintained for three children, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee shall obtain all required documentation for the three children and ensure the records are properly maintained in their files. Proof of compliance shall be submitted to the LPA by the due date.
