Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. During the inspcetion,LPA observed that this facility does not have a functioning carbon Monoxide detector, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to provide proof of the presence of a working carbon monoxide detector present at this facility by the POC date.
Based on observation,interview, and record review, the licensee did not comply with the section cited above. The review of drill log indicates that the last drill was conducted 01/24/2022. This poses a potential health and safety risk to children in care.
Resolution: Licensee shall submit proof of correction by the aforementioned date. Failure to follow through with this plan of correction will result in a civil penalty of $100 per day until corrected.
Based on record review, the licensee did not comply with this requirement as there was no file for C10 and C11, which poses a potential Health and Safety risk to children in care.
Resolution: Licensee shall obtain the missing forms by 2/06/2023 and mail a copy to Licensing as proof of correction.
