Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above.The facility is currently constructing an additional room adjacent to the living room( on- limit area), but the department was not notified of this modification which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Upon completion, the licensee shall notify the Department, and an inspection will be required. The facility shall also submit an updated facility sketch indicating whether the new room will be on-limits or off-limits to the children in care.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. Due to a medical emergency, the licensee had to go to the doctor and left the children in the care of S2, who did not have fingerprint clearance.This poses an immediate risk to the health, safety, or personal rights of the persons in care.

Resolution: The licensee shall process her helper's fingerprint clearance prior to being in the presence of children in care. The facility shall immediately exclude the helper lacking fingerprint clearance. Staff member was immediately removed from the facility. A civil penalty of $100 is being assessed.

Resolution: There should always be one individual with current pediatric First Aid and CPR training supervising the children. The facility is required to submit proof of enrollment in a CPR/First Aid class or a current certificate to the department for verification by due date.
