Based on observation, the licensee did not comply with the section cited above in fire extinguisher did not have service tag or receipt available which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have fire extinguihser serviced or purchase new and submit receipt or service tag to LPA via email as proof of correction by agreed upo date.

Based on observation, the licensee did not comply with the section cited above in that infant rocker was observed in daycare area which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility staff removed the infant rocker from the facility. Per licensee the staff will review the PIN 20-24-CCP infant safe sleep.

Based on record review, the licensee did not comply with the section cited above in the 2 out of 4 staff did not have record of Mandated Reporter Certificate AB1207 conliant which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the staff will complete the staining ad a copy of the certificate will be sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above in that 9 out of 12 children in care did not have record of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the immunization records for children will be obtained and a copy sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above for 11 month infant in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the LIC 9227, sleep plan will be completed by parent and sent to LPA via email by POC due date.

Based on observation, the licensee did not comply with the section cited above in that bathroom cabinet under the sink did not have a lock or safety latch and had ajax, soap and cleaners which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee placed a safety lock on the bathroom cabinet during inspection. The deficiency was corrected during visit.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that there were 12 children including 6 infants present at the same time which is over the ration allowed which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, two infants will be disenrolled. LPA observed 7 children in care during inspection. Per licensee, a wriiten plan will be provided including how the facility will maintain the ration and capacity by 5/20/24

Based on observation the licensee did not comply with the section cited above in that The facility did not have a crib or play yard for 11 month old Infant in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will ensure that there is a crib or play yard for infants in care that are not able to climb out. Infant was picked up by paretns during this inspection.

INFANT SAFE SLEEP(j)(2)(D)(c) based on records review LPA did not observe a sleep log for child #1,4 and 9 on file. This poses a potential health and safety risk to children in care.

Resolution: Licensee stated that she will provide proof by POC due date and will review Safe Sleep Regulations packet provided by LPA.

.8662 Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion (b) (1)  ... LPA did not observe staff #1 and #2 to have proof of certificate.This poses a potential health and safety risk to children in care.

Resolution: Licensee stated that she sent a text to the staff to complete training by 12/13/21. Licensee stated that she will be signing up to receive Provider Information Notices and use the entry checklist as a reminder.

.622 Employees or volunteers at family day care home; immunization requirements; records; exemptions.. based on records review LPA did not observe proof of immunization for staff #1, #2 and #3. This poses a potential health and safety risk to children in care.

Resolution: Licensee has requested proof of immunization be provided to licensee and she will provide LPA proof by POC due date. Licensee stated that she will be signing up to receive Provider Information Notices and use the entry checklist as a reminder. Licensee will create a checklist.
