Based on record review, the licensee did not comply with the section cited above in 1 of 2 staff (assistant) did not have mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have assistant complete required Mandated Reporter training for child care providers and submit certificate to LPA via email as proof of correction by agreed upon date.

Based on interview and record review), the licensee and two employees did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have all adults working with children completing the mandated online training for child abuse on or before 8/14/24 then submit to LPA in order to clear this citation.

Based on observation and interview, the licensee did not comply with the section cited above by allowing children to access the off-limits area (master bedroom, office room, and full bathroom which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licenesee corrected during today's visit. LPA obtained letter request and updated facility sketch to include the office room and full bathroom in the accessible area for daycare children.
