Resolution: Licnesee Martha will ensure she and all employees have a valid Mandated Reporter Training Certificate. Licensee Martha will provided proof via email of a valid Mandated Reporter Training Certificate.
Based on record review , the licensee did not comply with the section cited above Licensee Martha was missing proof of immunization for Measles which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure herself and all employee have required immunizations on file. Licensee will provide proof of measles by POC Due Date via email.
