Based on record review, the licensee did not comply with the section cited above in LPA observed an expired Mandated Reporter Training Certificate - 9/23/2021 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licnesee will ensure to have a current Mandated Reporter Training Certificate on file readily available. POC will be cleared by visit.
Based on observations and interview, the licensee did not comply with the section cited above in there was not an alarm in place which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure to have n alarm that, when placed in a swimming pool, will sound upon detecting an entrance into the water. The alarm shall be turned on and be in working condition during a facility’s operating hours while the swimming pool is not in use. POC will be cleared by visit.
Based on interview and interview, the licensee did not comply with the section cited above in a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard was not observed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard is visible and readily available. POC will be cleared by visit.
