Based on observation, the licensee did not comply with the section cited above in 1 out of 1 fire extinguisher had a purchase date of 8/11/23 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a new fire extinguisher and submit receipt as proof of correction to LPA via email by agreed upon date.

Based on record review, the licensee did not comply with the section cited above in Licensee had expired Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete Mandated Reporter training and submit certificate of completion to LPA as proof of correction via email by agreed upon date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 7 children did not have immunization records on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will get a copy of both childrens immunization records and submit to LPA as proof of correcetion via email by agreed upon date.

Based on record review, the licensee did not comply with the section cited above in staff #2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that going forward she will make a calendar as reminder when to retake trainings, a copy will be placed in assistant files and will make sure records are kept up to date,

Based on record review, the licensee did not comply with the section cited above with child #5 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that going forward she will make copies of the blank log to kepp in the children's files, train assistant, put it into our daily routine.
