Based on observation,interview, and record review, the licensee did not comply with the section cited above for unable to provide emergency drills log which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 4 out of 4 staff did not have current CPR and First aid which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above for not completed water testing for lead in the food-preparation sinks area used to wash children's fruits, which poses a potential health, safety or personal rights risk to persons in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.

Based on record review the licensee did not comply with the section cited above for staff #1, #2, & #3 which poses/posed a potential health, safety or personal rights risk to persons in care.

Result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance This requirement was not met as evidence by record review. Lead test results showed sink faucet kitchen sources had an action level exceedance. Per staff, the kitchen water faucet made inaccessible to children. Parents provide children lunch and snacks. Facility also have filtered water for children. This is a potential risk to the health and safety of the children in care.
