Based on observation, the licensee did not comply with the section cited above in LPA observed multiple electrical outlets within the facility uncovered and unsecured which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure facility is free from defects and conditions which might endanger a child. Licensee will ensure all electical outlets has an electrical outlet cover. POC will be cleared by visit.

Based on observation, the licensee did not comply with the section cited above LPA observed a fire extinguisher in the day care room with an expired service tag August 7,2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure to maintain a new or serviced fire extinguisher annually. Licensee will service fire extinguisher or purchase a fire extinguisher. POC will be cleared by a visit.

Based on observation LPA did not observe the gate on the mesh fence to self-close or self-latch when opened. LPA observed some tears in the fence which poses a potential health, safety or personal rights risk to persons in care. LPA observed the pool is located in the off-limit backyard (fenced). LPA tested the alarms in place to alert licensee that the sliding doors are open to sound on.

Resolution: LIcensee stated that she will make sure that the gate self-closes and self-latches.

Based on record review staff #2 is missing proof of Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will make sure that assistants complete training withing 30 days from starting.

Based on LPA observation of the Fire Extinguished service tag indicated that the last service date was 11/25/19. The licensee did not comply with the above requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the fire extinguisher will be serviced and a copy of the service tag will be provided to the department via email be POC due date.
