Based on observation, the licensee did not comply with the section cited above as LPA observed a rabbit inside the bathtub with rabbit droppings in the bathroom. Turtles in the living without a cover and green water. There are areas in the living room, dining room and bathroom with Amazon packages, paper and mail clutter piled in small stacks in these areas, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will clean animal droppings and turtle aquarium and place a secured cover over aquarium. LIcensee states she will remove cluttered areas and provide proof to LPA no later than plan of correction due date.

Based on observation and interview, the licensee did not comply with the section cited above in that there were cleaning products which state "keep out of reach of children" under the kitchen sink, which was not locked, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed products from being accessible, and Licensee stated she will keep the cleaning products out of reach of children until she installs a proper child lock on the cabinet, at which time she understands she can store the cleaning products under the sink as long as it is locked at all times when children are present in the facility. Licensee to submit proof of child lock installation by POC due date to CCL.

Based interview and record review, Licensee did not have copies of immunizations and did not have immunizations documented. LPA Mercado reviewed 4 out of 5 children files and two files did not have immunizations and were not documented. Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will send immunizations of child #1 and child #3 records and sumbit copies to Fresno Regional Office by POC due date.

Based on observation, LPA Mercado observed a car seat in the middle of the living room with a blanket inside the car seat and children have access to the living room. Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA did not observe a car seat in areas accessible to children. Licensee stated will no longer have car seat in areas accessbile to children and will place car seat outside or in off limits bedroom. Deficiency is cleared during todays inspection.

Resolution: Licensee stated will have all parents and new enrolling parents to fill out LIC 9224 and provide copy of licensing report.

Based on observation, LPA Mercado observed infant #1 in a pack in play in the dinning room with blankets while infant #1 was napping. Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will submit proof to Fresno Regional Office of no items (blankets, hats, etc) in pack n play while infant #1 is napping.

Based on interview and record review, Licensee stated conducts 15 mintue checks and logs information for infant #1 but could not produce log during inspection. Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will dociument 15 mintue checks for infant #1 and sumbit copy of log to Fresno Regional Office by POC due date.

Based on on observation and interview the licensee did not comply with the section cited above. LPA observed dirty dishes that piled the sink, the counters were cluttered and had many crumbs. The kitchen floor and counters were cluttered with food jars and other items making it difficult to properly clean and disinfect. LPA reviewed the requirement above and indicated that although the children did not have access to the kitchen, licensee reported she stores and cooks the children's meals in the kitchen, thus should be kept in a clean and orderly condition. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would clean her kitchen space in order to maintain a healthy and safe cooking enviornment for the children's meal preperation and storage. Proof of Correction to be provided to the Department by POC date 12/17/2021.

CRITICALHEALTHCCR 102417(g)(5)(B)Type ADec 3, 2021

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed that one of the two pool gates did not self latch as required. The licensee uses a key lock to maintan the pool gate closed. The pool gate faces a yard that is inacessble by gate to the children and has been documented to be off limits. Licensee confirmed this observation today. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fix the gate to ensure it is self latching. Today, licensee kept the lock on the gate and wrote a statment indicating her plan to ensure children continue not to enter the inacessible side yard where the non self latching gate is located. Proof of correction will be submitted by POC date 12/6/2021.
