Based on interviews conducted with director and staff, children age 2 from the Toddler Option are comingled with preschool children prior to transittion and at times to meet the ratio. This poses an immediate health and safety risk to children in care.

This requirement is not met as evidence by: According to interviews with S1 to S3, it was disclosed that on 08/20/2025 C1 was missing from room 2 for approximately 5 minutes and was found in room 3.
