Resolution: Per Licensee, Licensee and Staff will obtain current Mandated Reporter Training and provide proof to the department by the POC date.

Based on interview and observation, the licensee did not comply with the section cited above in LPA and LS observed a gun stored in upstairs off limits bedroom ingun case but there is no lock which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, assistant was only here today to help because she was not feeling well. Ms. Li stated that she was just here today to assist. Ms. Li left shortly. Licensee stated will have assistant get fingerprinted and will not return until clearance received. Licensee stated she understands fingerprint clearance requirement to ensure compliance.

Based on observation, the licensee did not comply with the section cited above in LPA and LS observed 7 children in care and 5 of whom are infants, licensee is over ratio by 1 infant which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, will talk to parent at pick up and will disenroll 1 infant by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA & LS observed fire extinguisher is fully charged but as not been serviced annually as required which poses/posed a potential health, safety or personal rights risk to persons in care.
