Based on record review, the licensee did not comply with the section cited above in director and staff have current certificate, but the certificate is from an entitly that is not EMSA approved. No EMSA sticker on certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed Staff 1 (S1) is not associated to the facility. LPA observed on LIS that S1 has a fingerprint clearance. Per Director, she didn't submit the transfer request form. This poses an immediate risk to the health, safety, and personal rights of child in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.

Based on interview and record review, it was disclosed by 3 of 7 staff, that a child was left unattended in a classroom without supervision on 11/19/2021. This posed an immediate risk to the health, safety, and personal rights of child in care.

Based on interview, Staff #1 stated that the incident where a child was left unsupervised in a classroom, took place, but facility did not make any report to the Department. This poses a potential risk to the health, safety, and personal rights of children in care.
