Based on interview and record review, the licensee did not comply with the section cited above as S1 did not have proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of S1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/12/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 and the Licensee were missing proof of the pertussis immunization which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1 and Licensee's pertussis immunizations. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/12/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing proof of the Individual Infant Sleeping Plan (LIC9227) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1’s authorized representative complete the LI9227 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/12/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 and C2 were missing proof of the 15 minute sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document 15 minute sleep checks for C1, C2, and all infants enrolled in care. Licensee agrees to send proof of the 15 minute sleep checks for C1 and C2 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/12/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed a damaged child sized toy car which broken pieces and jagged edges in the on-limits backyard as well as a bicycle which was missing the handle bar protectors which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/12/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee and Assistant Provider were missing proof of current non-expired the Mandated Report Child Care Provider's training (AB1207) as Licensee's and Assistant Provider's previous AB1207 expired on 04/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete themselves and have Assistant Provider complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of Licensee's and Assistant Provider's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/27/2024.
