Based on observation and interview, the licensee did not comply with the section cited above as LPA was unable to verify that ammunition is stored and locked separately from firearms as Licensee did not have the key to show LPA the firearms and ammunition which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit photos of ammunition being stored and locked separately from firearms. Licensee also agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/12/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain proof of C1's required immunizations. Licensee agrees to send proof of C1's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/12/2025.
Based on observation, the licensee did not comply with the section cited above as LPA observed an above ground pool present that had water inside it and observed a five-foot mesh fencing along one side of the pool that did not prevent direct access from the windows and sliding glass door of the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to either remove the bodies of water or agrees to come into compliance with the above listed regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/20/2024.
Based on interview and record review, the licensee did not comply with the section cited above as Licensee did not have proof a current, not expired, Mandated Reporter Child Care Providers (AB1207) training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers Training (AB1207) and agrees to send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/31/2023.
Based on LPA observation- the licensee did not comply with tthe section above : At 8:15AM, LPA observed kitchen area (on limits) in process of renovations with exposed cabinets, wires, pipes which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee must make the entire kitchen inaccessible by POC 09/25/2021. Licensee will submit proof of completed renovations of kitchen once completed. Licensee submitted updated sketch for kitchen making it off limits.
Based on LPA observation the licensee did not comply with the section cited above: At 8:35am LPA observed 2 broken tricycle pedals and cracked plastic on climbing play structure in front yard which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of repaired pedals or remove tricycles completely. Licensee will submit proof of play structure repair and not allow usage of play structure until repaired. All repairs will be completed by POC due date of 10/01/2021
