Based on observation, the licensee did not comply with the section cited above as LPA observed several hazards such as broken wood and plastic items in the backyard and button batteries in the living room which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee moved items to inaccessible areas during the visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/14/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of the Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/14/2025.

Based on interview and record review, the licensees did not comply with the section cited above as Licensees could not provide proof of the documentation of 15 minute sleep checks for C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to conduct and document 15 minute sleep checks by the Plan of Correction (POC) due date of 08/16/2023. Licensees agree to submit documentation of 15 minute sleep checks for C1 to Community Care Licensing (CCL) by the end of the business day on the POC due date of 08/16/2023.

Based on interview and record review, the licensee did not comply with the section cited above as Licensees were unable to provide proof of the MMR vaccine for both Licensees which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to obtain proof of the MMR vaccine for themselves and submit proof of the MMR vaccine by the Plan of Correction (POC) due date of 08/16/2023. Licensees agree to submit documentation of the MMR vaccines for Esther Sanchez and Arturo Hernandez to Community Care Licensing (CCL) by the end of the business day on the POC due date of 08/16/2023.
