Resolution: Licensee agrees to document 15 minute sleep checks for C1 and all infants enrolled in care. Licensee agrees to send proof of the 15 minute sleep checks for C1 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 03/10/2026.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee and S2 were missing proof of current Mandated Reporter Child Care Providers training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and have S2 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of Licensee's and S2's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 03/10/2026.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 and C4 were missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C2 and C4's required immunizations. Licensee agrees to send proof of C2 and C4's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 03/10/2026.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of measles and pertussis immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1's measles and pertussis immunizations. Licensee agrees to send proof of S1's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 03/10/2026.

Based on record review, the licensee did not comply with the section cited above in that one out of eight children’s records are not complete. C6 file is missing the Individual Infant Sleeping Plan (LIC 9227) which poses a potential health and safety risk to persons in care.

Resolution: Licensee agrees to submit and maintain an Infant Individual Sleeping Plan on file for any infant under the age of 12 months. Licensee agrees to submit completed LIC 9227 for C6 to Community Care Licensing (CCL) by the POC due date of 11/21/2024.

Based on record review, the licensee did not comply with the section cited above in that the roster was not completed upon initial review, which posed a potential health and safety risk to persons in care.

Resolution: Licensee completed roster during inspection.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as LPA observed seven children present, all who are under the age of 6 and are not enrolled in kindergarten or are school age children, with only one qualified adult caring for them which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to write a written statement detailing their plan to stay in ratio at all times, especially when Licensee is absent from the facility. Licensee agrees to submit proof of this Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 10/12/2023.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee stated they have been physically conducting 15 minute checks for C2, but they have not been documenting the 15 minute sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to begin physically documenting the 15 minute sleep checks for C2 and any other children enrolled who are under the age of two years old. Licensee agrees to submit proof of this Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 10/25/2023.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee’s Mandated Reporter Training expired on 09/2021 and S2’s Mandated Reporter Training expired on 07/2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and renew the Mandated Reporter Child Care Providers training (AB1207) for both themselves and S2. Based on interview and record review, the licensee did not comply with the section cited above as the last fire and disaster drill was conducted 01/2023, past the every six month stipulation, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a new fire and disaster drill with daycare children to comply with regulations. Licensee agrees to submit proof of this Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 10/25/2023.
