Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that consent to administer IMS services was missing for C7 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C7’s authorized representative complete a consent form to administer IMS services and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/17/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed a tricycle with missing handlebars and another tricycle with a missing seat which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee moved items during the inspection to an inaccessible area. POC cleared during the visit.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of the Mandated Reporter Child Care Provider training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/17/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1, C2, and C3 were missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C1, C2, and C3's required immunizations. Licensee agrees to send proof of C1, C2, and C3's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/17/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 and S2 were missing proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1 and S2's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of S1 and S2's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/17/2025.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed two hand lighters, 2 propane tanks, and various gardening tools outside in the on-limits backyard area which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to store away hazards and agrees to write a plan of action in order to ensure compliance. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/14/2024.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed two empty Marlboro cigarette packs, a cigarette butt, and two hand lighters in the in-limits backyard area which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to read the Smoking Prohibition regulations and agrees to write a plan of action to ensure compliance with regulations. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/14/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of the Pertussis vaccine which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1's Pertussis vaccine and maintain proof in S1's file. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/14/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2, C6, C8, and C10 were missing proof of immunizations for their age which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C2, C6, C8, and C10's immunizations and agrees to update children's immunizations as specified in the regulations. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/14/2024.

Resolution: Licensee agrees to conduct 15 minute sleep checks as required in the Infant Safe Sleep Regulations and maintain proof in children's files. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/14/2024.
