Based on observation and interview, the licensee did not comply with the section cited above as LPA observed several hazards in the on-limits backyard such as damaged toys, damaged play equipment, and dog feces which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/20/2025.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed that the carbon monoxide detector was not plugged in which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee plugged in the carbon monoxide detector and tested the detector during the visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/20/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 and C4 were missing proof of 15 minute sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document 15 minute sleep checks for C2 and C4. Licensee agrees to send proof of the 15 minute sleep checks for C2 and C4 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/20/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed several hazardous items present in on-limit areas in the reach of children such as knives, a scissor, a razor, and several items with "keep out of reach of children" labels, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained in regard to the above listed regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/17/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee did not have a current fire/disaster drill on file and Licensee stated they did not know where the drill log was which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to either obtain proof of latest fire/disaster drill or agrees to conduct and document a new fire/disaster drill if proof cannot be obtained. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/30/2024. A civil penalty of $250.00 was assessed.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee was unable to provide proof of a valid Mandated Reporter Child Care Providers (AB1207) training certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers (AB1207) training by the Plan of Correction (POC) due date of 08/25/2023. Based on interview and record review, the licensee did not comply with the section cited above as Licensee stated that the last firedrill was conducted sometime last month but that the fire drill log has since been lost which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill and agrees to document said fire/disaster drill by the Plan of Correction (POC) due date of 08/25/2023. Licensee agrees to send proof of the fire/disaster drill and documentation to Community Care Licensing (CCL) by the end of the business day on the POC due date.
