Resolution: Licensee agrees to obtain proof of S1's measles and Pertussis immunizations. Licensee agrees to send proof of S1's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 04/01/2025.

Based on record review, the licensee did not comply with the section cited above as LPAs observed that the last drill documented was 06/05/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to either obtain proof of latest fire/disaster drill or agrees to conduct and document a new fire/disaster drill if proof cannot be obtained. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that proof of current Mandated Reporter Child Care Providers training (AB1207) was missing for Licensee and S1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of Licensee's and S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 04/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that Licensee and S1, who was left alone prior to LPAs arrival, were missing proof of current non-expired EMSA approved CPR/1st Aid which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of Licensee's and S1's non-expired EMSA approved CPR/1st Aid Card or agrees to enroll in an EMSA approved CPR/1st Aid course if proof cannot be obtained. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that C2 and C3 were missing proof of the Family Child Care - Notification Of Parents' Rights (LIC995A) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C2 and C3’s authorized representative(s) complete the LIC995A and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/01/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as upon arrival LPAs observed Licensee arrived with their own biological child at the same time as LPAs and S1 was observed alone with 7 children in the home of which none of the children were at minimum six years old or were enrolled in school which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with staffing ratio and capacity. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/01/2025.

Based on interview, the licensee did not comply with the section cited above as Licensee stated that they serve all children in care, regardless of age, whole milk which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof of purchase of 1% milk. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/27/2025.
