Type A citation from complaint investigation - immediate risk

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed director and assistant director had expired mandated reporter certificationsh ich poses/posed a potential health, safety or personal rights risk to persons in care.

.8662(b)(1) - Based on staff file reviews by LPA, of the 25 staff files reviewed 20 were missing current mandated reporter certificates. This is a potential health and safety risk to children.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed staff #1 (S1) did not have immunization records inside their file and could not provide proof of immunization records during the visit which poses/posed a potential health, safety or personal rights risk to persons in care.

.7995(a)(1) - Based on staff file review by LPA, of the 24 files reviewed 4 did not have proof of immunization against pertussis, measles, and influenza. This is a potential health and safety risk to children.

(2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This report shall indicate the following:

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
