Based on the observation and inspection, the licensee did not comply with the section cited above in putting shampoo and cleaning materials in the unlock under sink cabin at the bathroom which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the shampoo and cleaning materials right away and put in the storage. Licensee will provide a plan of correction and photo to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in 3 out of 6 children files does not have parent's signature on LIC 995A form which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have parents sign on LIC 995A form right away. Licensee stated for new enroll student, will make sure parent sign the form and give parent a copy of signed form.
