Based on record review, S1 and S2 are missing the required immunizations. S1 is missing immunizations in measles and pertussis and S2 is missing immunizations in measles, pertussis, and flu which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date: 02/04/25, Licensee states she will submit proof of immunizations for the two helpers.

Based on LPA's observation, Licensee retrieved three daycare children (C1, C2, & C3) who were across the street from licensee's home unattended which poses an immediate health, safety or personal rights risk to persons in care. Since absence of supervision is a zero tolerance violation, an immediate civil penalty of $500 was assessed.

Resolution: By POC due date: 01/15/25, Licensee states she will submit a written plan on how she will ensure that children are supervised at all times.

Based on LPA's observation and licensee's statement, Ilaha Rzayeva, has been working in the home for a month without fingerprint clearance. This poses an immediate risk to the health, safety or personal rights to children in care. Civil penalty of $500.00 was assessed.

Resolution: Licensee stated that Ilaha Rzayeva cannot return to work until she receives her fingerprint clearance. Licensee's written Plan of Correction was received during the inspection. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.

Based on record review, the licensee did not comply with the section cited above by not maintaining documentation of current fire/disaster drill which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will practice fire/disaster drill with the children and will submit documentation by November 4, 2021.

Based on record review, child #2, #3, #6, and #7 are missing the signed Notification of Parents' Rights (LIC 995A) in their files. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit completed and signed LIC 995A for the four children by November 4, 2021.

Based on record review, licensee's helper (Helper #1) has been working in the home for more than 90 days, however, there was no proof of completed Mandated Reporter Training for Child Care Providers in her file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will have her helper complete the training and will submit proof of completion by November 22, 2021.

Based on record review, licensee states that she practiced fire/disaster drill with the children, however, she failed to present documentation of the drill. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will practice fire/disaster drill with the children and submit documentation to Licensing by November 4, 2021.
