Based on record review, the licensee did not comply with the section cited above. LPA did not observe a Safe Sleep log for all infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Safe Sleep log for all infants in care from 09/30/2024 through 10/04/2024 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 4 is missing LIC995 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC995 for child 4 to CCLD by POC date.

Based on observation, interview and record review, the licensee did not comply with the section cited above, Licensee was alone with 4 infants and 8 preschool age children until 9:52 AM when assistant A1 arrived to assist which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Deficiency was cleared during site visit, A1 arrived to help licensee with children in care, licensee had also called parents to pick up a children in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in fire extinguisher was purchased on 07/05/21 and has not been serviced, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of service or purchase of approved fire extinguisher to the San Jose Regional Office by close of business 4/04/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Provider did not have documentation of conducting fire and disaster drills every 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of fire and disaster drill performed to the San Jose Regional Office by close of business 03/24/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above in infant C1, was left in infant room crying with door almost fully closed, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee cleared deficiency during site visit, door was proped open with sandal and child removed to day care area.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Child C3 missing from current roster, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee updated child care roster during site visit.

Based on observation, interview and record review, the licensee did not comply with the section cited above, fitted sheets were not being utilized on mattress with sleeping infant C3, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a photo of cribs with fitted sheets being used to the San Jose Regional Office by close of business 3/24/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above in, aerosol can of Lysol, Clorox spray bottle, liquid Cleaner degreaser in accessible unlocked kitchen sink cabinet and hand soap in unlocked on limit bathroom cabinet which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee corrected deficiency during site visit, bathroom sink cabinet was locked and kitchen area was made inaccessible by closing day care room with baby gate.

Based on record review, the licensee did not comply with the section cited above by having a Mandated Reporter Training certificate that had expired on 09/12/2021, which poses a potential health, safety or personal rights risk to persons in care.
