Type A citation from complaint investigation - immediate risk
Based on record review, the licensee did not comply with the section cited above, facility has not completed lead testing in accordance to the Written Directives which poses/posed a potential health, safety or personal rights risk to persons in care.
Type B citation - potential risk if not corrected
On 05/25/21, during an interview with Director Narissa Groves, by her own submission stated that there were three cases of head lice in the facility of children in care and was not reported to the Department. This poses a potential risk to the health and safety of children in care.
Based on record review, the licensee did not comply with the section cited above, facility has not completed lead testing in accordance to the Written Directives which poses/posed a potential health, safety or personal rights risk to persons in care.
There were collaborating statements made by Director, S1 and S2 that C3, C4, C5, C6, C7 had head lice on May 13, 2021. In which the director was aware of three, C3, C4, C5, the director was not aware of two, C6, C7. Another statement by C1, who stated that the teacher showed him the lice. This poses an immediate risk to the health and safety of children in care.
