Based on [observation], the licensee did not comply with the section cited above the fire extinguisher was not serviced yearly, has a purchase receipt dated 11/10/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Amparo B stated the fire extinguisher will be serviced and a copy of the service tag will be emailed to LPA by POC due date.

Resolution: Licensee stated they will purchase a new fire extinguisher size 2A 10BC. Licensee will email verification of purchase to LPA by POC due date.
