Based on observation, the licensee did not comply with the section cited above, The living room and dining room was observed to be cluttered varies items on the floor such as roaster ovens, instrumental tripods, keyboard and stool which poses/posed a potential health, safety or personal rights risk to persons in care. In addition, LPA’s observed varies items stacked against the wall in the outside patio.
Resolution: Per licensee, will clear all items in the living room, dining room and outside patio by POC due date.
Based on interview, the licensee did not comply with the section cited above diaster drill was not conducted every six months which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee will conducted drill by POC due date.
Based on record review, the licensee did not comply with the section cited above in 1 out of 4 files did not have the LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee will insure Child #1 has the LIC 9227 Infant Sleep plan by POC due date.

Resolution: Per licensee will maintain 15 minute sleep log for Child #1, Child #2, Child #3 and Child #4 and will submit verification by POC due date.
Based on observation, the licensee did not comply with the section cited above in, mandated reporter training certificate has expired for licensee which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee will submit mandated reporter certificate to LPA by due date.
