Based on observation, interview, and record review, the licensee did not comply with the section cited above, by not providing a copy of the accusation to parents, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit statement indicating parents were provided a copy of the accusation served to licensee on 7/30/25, and copies of the LIC9924's signed by parents to CCL by 2/25/26. nicolette.cunningham@dss.ca.gov
Based on observation the licensee did not comply with the section cited above in which LPAs toured the facility and observed in the kitchen, a bowl of food scraps on the counter crawling with gnats in the area that is used to prepare children's meals which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will take food scraps to their guinea pigs and provide LIC 9224 to parents and copy of licensing reports, provide documentation to CCLD by 3/7/25
