Based on LPA observation and interview, the licensee did not comply with the section cited above as LPA observed a broken fence pole, missing pieces of privacy fence, and broken toys/equipment which poses a potential health, safety or personal rights risk to persons in care.

Based on staff record review the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Of the 12 staff files reviewed 11 did not have current Mandated Reporter Training certificates to review.

Based on LPA observation and interview the licensee did not comply with the section cited above as LPA observed a torn carpet (approximately 1 inch X 10 inches), dirt on the diaper changing table, power outlet without covers, and scraped wall with plaster exposed approximately 2 inches in diameter which poses a potential health, safety or personal rights risk to persons in care.

Based on observation of the outdoor play area of the preschool, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. It was observed that some of the plastic play structures had holes and were in disrepair.
