Based on LPA observation and interview, the licensee did not comply with the section cited above as LPA observed a broken fence pole, missing pieces of privacy fence, and broken toys/equipment which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation and interview the licensee did not comply with the section cited above as LPA observed a torn carpet (approximately 1 inch X 10 inches), dirt on the diaper changing table, power outlet without covers, and scraped wall with plaster exposed approximately 2 inches in diameter which poses a potential health, safety or personal rights risk to persons in care.
